Strengthening Michigan Medicaid Programs So Adults Can Receive Care Safely in Their Homes Rather than in Institutions

When services are well‑designed and effectively delivered, adults with complex health needs can live more safely and comfortably at home. National evidence shows that at‑home acute care is associated with lower mortality rates, reduced post‑discharge spending, and positive experiences for patients and caregivers.

In 2025, we supported Michigan Medicaid home and community‑based services by strengthening oversight and accountability through audits of two of Michigan’s Medicaid c-waiver programs: MI Choice and MI Coordinated Health (MICH).

The MI Choice program helps eligible adults receive services at home and in their communities instead of in nursing homes. The MI Coordinated Health program serves adults who qualify for both Medicare and Medicaid by coordinating their medical care, behavioral health care, prescriptions, and other supports. Through its waiver program, MI Coordinated Health also provides additional services that help people live safely at home rather than in institutional settings.

MPHI staff conducted record reviews and in‑person interviews, with scoring aligned with regulatory requirements to support programs serving more than 18,000 Michiganders. This work strengthened program oversight and contributed to improved service quality and outcomes for individuals receiving care at home. Results from auditing activities conducted in 2025 show that every Integrated Care Organization improved compliance from the previous year in at least one area, and aggregated scores improved in multiple areas over the same period.

Through audits and direct beneficiary engagement, MPHI teams helped ensure services were delivered safely and consistently, in alignment with program standards.
